Start by submitting the IDDW application to Acentra Health

Save the application and submission proof. If no response arrives within five business days, call Acentra. When the approved-psychologist packet arrives, return your choice within 14 days, schedule the evaluation, and track medical eligibility, DoHS financial eligibility, available program space, enrollment, planning, and delivery separately.
