Oregon’s Community Developmental Disabilities Programs receive I/DD applications and determine eligibility. After eligibility, an adult may receive case management from a CDDP services coordinator or, when the living-setting rules allow, choose a Support Services Brokerage personal agent.

Not started or approaching school exit

Open the CDDP eligibility route, build the record, and connect school and VR work.

  2. Use case-management referrals and applicable official provider, licensing, site, or vacancy resources, then verify current facts directly.

A decision is adverse or decision support is needed

Use the correct hearing, complaint, benefits, and decision-support routes.

  2. Use the current complaint route for service, treatment, staff, planning, or process concerns without losing a separate hearing or protective-report route.
  3. Keep Social Security work moving while documenting the adult's choices and using supported decision-making when wanted.

Urgent need, crisis, abuse concern, or service breakdown

Address immediate safety, then document and correct the system failure.

  1. Call 911 for immediate danger or a medical, police, or fire emergency; otherwise call, text, or chat 988 for crisis support.
  2. Report suspected abuse of an adult with I/DD through Oregon's statewide route while separately addressing service failure.
  3. Document changed needs, failed supports, and risk and request the specific ONA review, ISP change, exception, authorization, provider correction, or crisis action.
