Start with the local Community Developmental Disabilities Program. After eligibility, use the chosen CDDP services coordinator or Support Services Brokerage personal agent. For an adverse action, use the exact notice and the current complaint or hearing route.
Step by step
- For immediate danger or a suspected crime requiring emergency response, call 911.
- Report suspected abandonment, neglect, seclusion, wrongful restraint, financial exploitation, or physical, sexual, or verbal abuse through 855-503-SAFE.
- Separately notify the case manager and provider and use complaint, hearing, licensing, or service-correction routes according to the issue.
