Who to contact
Start with the local Community Developmental Disabilities Program. After eligibility, use the chosen CDDP services coordinator or Support Services Brokerage personal agent. For an adverse action, use the exact notice and the current complaint or hearing route.
Step by step
- Prepare goals, non-negotiables, risks, choices, and questions.
- Review every service, amount or hours, provider or worker role, natural support, safeguard, owner, schedule, and backup.
- Obtain the completed ISP and separately confirm authorization, provider or worker onboarding, and start.
