Who to contact
Start with the local Community Developmental Disabilities Program. After eligibility, use the chosen CDDP services coordinator or Support Services Brokerage personal agent. For an adverse action, use the exact notice and the current complaint or hearing route.

- Describe actual support needs and risks, including unpaid help and what happens when support is absent.
- Request a copy, review material facts and service-group result, and raise errors with the assessor or case manager.
