Residential provider profiles and vacancy reports

Before you start

  • Exact authorized service, setting, geography, schedule, accessibility and communication needs, health and behavior supports, risks, transportation, staffing, backup needs, and quality questions.

Who to contact

Start with the local Community Developmental Disabilities Program. After eligibility, use the chosen CDDP services coordinator or Support Services Brokerage personal agent. For an adverse action, use the exact notice and the current complaint or hearing route.

Find a CDDP or brokerage

Step by step

  1. Ask the services coordinator or personal agent for candidates appropriate to the ISP and authorization.
  2. Review applicable license, certification, endorsement, site, profile, home-screening, or vacancy evidence.
  3. Ask each candidate about intake, capacity, staffing, safeguards, experience, corrective history, backup coverage, and fit.

Expected next

A documented candidate list, direct responses, selected provider or worker, onboarding event, or written access gap.

Save this proof

Candidates, source links and dates, direct responses, licensing evidence, questions, referrals, onboarding, and dates.

If this stalls or the answer is no

Ask the case manager for more choices, document the access gap, and use complaint or licensing routes when appropriate.
