Ask for an exception to I/DD service limits

Before you start

  • Current ONA and service group when applicable, ISP and authorization, dated change summary, incidents, hospital or crisis contacts, caregiver changes, failed staffing, risks, requested response, and backup needs.

Who to contact

Start with the local Community Developmental Disabilities Program. After eligibility, use the chosen CDDP services coordinator or Support Services Brokerage personal agent. For an adverse action, use the exact notice and the current complaint or hearing route.

Step by step

  1. Send a concise dated change-and-risk summary to the services coordinator or personal agent.
  2. Request the specific ONA review, ISP change, exception to service-group hours or other limit, authorization, provider correction, or crisis action.
  3. Ask for an interim safety plan, written result or notice, named owner, and next-event date.
