Approaching school exit; adult services not started

DDD eligibility, the NJCAT, school transition, Medicaid, and vocational rehabilitation are connected but separate actions.

JNY-NJ-02

DDD eligible; Medicaid or program status unresolved

DDD functional eligibility, Medicaid eligibility, Supports Program enrollment, Community Care Program status, level of care, and service authorization are different decisions.

Enrolled; planning and authorization underway

The person-centered plan, NJCAT tier, annual budget, service definition, rate, authorization, and actual service start must line up.

JNY-NJ-04

Choosing providers or self-directed services

Provider discovery, direct capacity verification, model choice, service authorization, and rate implementation require separate checks.

JNY-NJ-05

Eligibility, Medicaid, or support decision is denied or uncertain

DDD eligibility appeals, Medicaid fair hearings, planning disputes, and parallel employment, housing, benefits, and decision-support routes have different owners.

JNY-NJ-06

Urgent need, crisis, abuse concern, or service breakdown

Immediate emergency response, behavioral crisis stabilization, abuse reporting, service correction, and reassessment are separate actions.
