Before you start

  • Current daily-support needs, self-care, behavioral and medical supports, communication needs, examples across settings, and people who can accurately describe support needs.

Who to contact

The assigned DDD facilitator; after eligibility, the Support Coordinator or DDD.FeeForService@dhs.nj.gov for a copy of the NJCAT answers.

Step by step

  1. Respond to scheduling and identify who should participate.
  2. During the NJCAT, give concrete examples of the support actually needed rather than only what the person can do in ideal conditions.
  3. Save the tier letter and request the question-and-answer copy when useful for planning.

Expected next

DDD states it mails the tier letter within four weeks after the face-to-face NJCAT.

If this stalls or the answer is no

Ask the facilitator or DDD for the assessment status, tier-letter date, and route to correct inaccurate information or request reassessment.
