A decision is adverse or authority is uncertain

Use the controlling notice while benefit and decision-support work continues.

  2. Use the appeal instructions in the complete Medicaid notice immediately and ask about continued benefits when an existing service is ending or being reduced.
  3. Keep Social Security work moving while documenting supports and considering less restrictive decision-help before guardianship.

Urgent need, crisis, abuse concern, or service breakdown

Address immediate safety, then document and correct the system failure.

  1. Call 911 for immediate medical or safety danger; otherwise call or text 988 or call GCAL at 1-800-715-4225, then notify the DD team.
  2. Use the setting-specific official abuse-report route while separately notifying the provider and DD team about service failure.
  3. Document changed needs, failed supports, and current risk and ask the Regional Field Office or support coordinator to update the applicable assessment, Planning List priority information, ISP, or crisis response.
