- For immediate danger call 911.
- For a community setting, report to APS online or at 1-866-552-4464, press 3; for a covered facility, use Healthcare Facility Regulation at 1-800-878-6442.
- Separately notify the provider, support coordinator, and Regional Field Office of the service breakdown and request corrective action.
