  2. Open Individual IDD Connects from the official application page and create an account, or download the paper application.
  3. Complete and submit the application and required documents; starting an online application is not the same as submitting it.

Regional staff review the submission, contact the applicant about missing material, and route a complete application for pre-eligibility review.
