A decision is adverse or authority is uncertain

Urgent need, crisis, mistreatment concern, or service breakdown

Address immediate safety, then document and correct the system failure.

  2. Report suspected mistreatment or self-neglect to the county Adult Protective Services intake while separately addressing provider or program failure.
  3. Document changed needs, failed supports, and risk and request the specific reassessment, plan revision, authorization, provider correction, or crisis action.
