Ask for DDD developmental-disability eligibility and adult-service intake. Save the contact, dated record index, every missing-record request, eligibility result, Case Manager assignment, and stated next event. Waiver admission is a separate process involving level of care, Medicaid, Long Term Care eligibility, and an MQD decision.
