Before you start

  • Complete notice if one exists, issue summary, dates, requested correction, service plan, budget and authorization documents, provider or worker records, prior contacts, representative authority, and current risk.

Who to contact

Start with the local ADRC or Tribal ADRS. After enrollment, use the selected MCO care team or IRIS consultant agency and fiscal employer agent. For an adverse action, use the exact notice and program-specific rights route.

Step by step

  1. Classify the issue as a grievance, adverse-benefit appeal, payment problem, provider or worker issue, or protective concern.
  2. Use the current IRIS grievance form or the appeal instructions and state fair-hearing form that match the issue.
  3. Preserve proof, request the record, and keep protective reports or urgent safety action separate.
