Long-Term Care Functional Screen overview

Before you start

  • Current screen result, member-centered plan or IRIS service plan and budget, dated change summary, incidents, hospital or crisis contacts, caregiver changes, failed staffing, risks, requested response, and backup needs.

Who to contact

Start with the local ADRC or Tribal ADRS. After enrollment, use the selected MCO care team or IRIS consultant agency and fiscal employer agent. For an adverse action, use the exact notice and program-specific rights route.

Step by step

  1. Send a concise dated change-and-risk summary to the responsible screener, MCO care team, or IRIS consultant.
  2. Request the specific functional-screen review, member-centered-plan change, IRIS budget amendment, authorization, provider correction, or crisis action.
  3. Ask for an interim safety plan, written result or notice, named owner, and next-event date.

An urgent contact, reassessment, plan or budget action, authorization response, crisis referral, corrective response, or written decision.
