Urgent need, crisis, abuse concern, or service breakdown

Address immediate safety, protective reporting, reassessment, and service correction.

  1. Use 911 for immediate danger and call or text 988 for a mental-health or substance-use crisis.
  2. Report suspected abuse, neglect, or exploitation to APS or law enforcement and separately correct the service plan.
  3. Document changed need, failed supports, or risk and request the exact reassessment, plan, authorization, provider, or priority response.
