- Immediate safety, person and location, setting, alleged conduct, dates, witnesses, injuries or losses, responsible service, prior reports, and requested correction.
Who to contact
DHS OIG, Adult Protective Services, law enforcement when applicable, provider supervisor, and assigned ISC.
Step by step
- Use 911 for immediate danger.
- For a DHS-funded, licensed, certified, or operated setting, review the OIG route at 1-800-368-1463; for a person with disabilities age 18-59 living in the community, review APS at 1-866-800-1409.
- Separately notify the ISC and provider about the service failure and request a backup or correction plan.
