Before you start

  • Current assessment, PCSP and PAN, dated change summary, incidents, hospital or crisis contacts, caregiver changes, failed staffing, risks, requested response, and backup needs.

Who to contact

Use the official destination above, the assigned DDCS case manager, a DDCS office, the county Service and Information Request line, or the exact contact on the controlling Planned Action Notice. Ask which unit owns the step.

Find an official DDCS office

Step by step

  1. Send a concise dated change-and-risk summary to the case manager.
  2. Request the specific significant-change or interim assessment, Plan Amendment, authorization review, provider correction, or crisis action.
  3. Ask for an interim safety plan, PAN or other written result, named owner, and next-event date.

Expected next

An urgent contact, assessment, Plan Amendment, PAN, authorization action, crisis referral, corrective response, or written decision.

Save this proof

Change summary, delivery proof, incident and medical records, contacts, interim plan, assessment, PCSP amendments, and PAN.

If this stalls or the answer is no

Use 911 or 988 for immediate danger; otherwise escalate within DDCS and preserve complaint and appeal routes.
