Review DC service coordination and ISP roles

Before you start

  • Current needs and risks, strengths and preferences, daily routines, health and communication needs, employment and community goals, current supports, and unmet needs.

Who to contact

Assigned DDA Service Coordinator; the Service Planning and Coordination Division owns planning and monitoring.

Step by step

  1. Ask the Service Coordinator to schedule or confirm the person-centered planning process.
  2. Document what is important to and for the person, the outcomes sought, and each support required.
  3. Review the written ISP for accuracy, responsible parties, and unresolved service or funding decisions.

Expected next

A written ISP recording decisions and agreements from the person-centered planning process.

Save this proof

Meeting notice, attendance, assessments, draft and final ISP, signatures or documented agreement, and unresolved-items list.

If this stalls or the answer is no

Ask the Service Coordinator to identify the policy step, missing assessment, or unresolved decision blocking the ISP; escalate to the supervisor when necessary.
