- The current priority or service plan, dated crisis or breakdown evidence, caregiver-capacity changes, and records showing the effect on health, safety, or community living.

The support coordinator and CSB/BHA own the relevant waiting-list or service-plan review.
Step by step
- Send the support coordinator a dated written summary and supporting records.
- Request review of the Priority Needs Checklist when the person is waiting, or the Individual Support Plan and authorization when enrolled.
- Ask for written confirmation of what review was opened, who owns it, and the next decision or meeting date.
Expected next
Confirmation that the new evidence is in the record and a scheduled review, revised plan, or written priority result.

Change packet, delivery confirmation, review request, meeting notice, revised checklist or plan, and written decision.
If this stalls or the answer is no
Escalate to the support-coordination supervisor and ask for the exact review route; use the notice’s appeal information if an adverse decision is issued.
