Step by step

  1. For physical danger, a life-threatening situation, a weapon, overdose, or an immediate emergency, call 911.
  2. For emotional, mental-health, suicide, or substance-use crisis support, call or text 988; state that you are in Utah if the phone number has a non-Utah area code.
  3. After immediate safety, notify the current DSPD owner and provider and request documented follow-up and plan review.

Expected next

A crisis response plus a named follow-up owner, referral, assessment, plan update, or service correction.

Save this proof

Contact times, responders, event facts, discharge material, notifications, referrals, and updated plan.

If this stalls or the answer is no

Escalate according to immediate risk and ask crisis and DSPD teams to name the post-crisis owner.
