Step by step
- For immediate danger or a suspected crime requiring emergency response, call 911.
- For a community-setting adult report, use DSS APS at 1-888-CARE4US or its online route; for an OIDD-operated or contracted residential setting, follow OIDD's SLED reporting instruction.
- Separately notify case management and the provider and use complaint, reconsideration, appeal, or oversight routes according to the service issue.
