Person-centered planning and HCBS setting rights

Before you start

  • Eligibility and program status, needs assessment, strengths, preferences, communication, goals, health and behavior needs, risks, desired services and settings, provider preferences, schedules, safeguards, transportation, natural supports, and backups.

  2. Review each assessed need, outcome, service, amount, provider role, safeguard, owner, schedule, and backup.
  3. Obtain the completed plan and separately confirm authorization, provider acceptance, staffing, and start.
