Step by step

  1. Contact the supports coordinator when needs change rather than waiting for the annual PUNS review.
  2. Describe the change, timing, unmet service, and health or safety consequences with supporting records.
  3. Ask to update PUNS and any affected plan information and retain confirmation of what was recorded.
  4. If the PUNS information is disputed, use the official disagreement form.
