Report vulnerable-adult abuse or neglect in Oklahoma

Before you start

  • Adult's location, vulnerability, immediate safety, observable facts, dates, injuries or risks, alleged responsible person, witnesses, current services, failed safeguards, and communication needs.

Who to contact

Use the official Liberty intake, DDS, case manager, SoonerCare, provider, hearing, grievance, crisis, or protective-report route that owns this step.

Use Oklahoma DDS intake

Step by step

  1. For immediate danger, call 911.
  2. Report suspected abuse, neglect, self-neglect, or exploitation online or to 1-800-522-3511 when applicable.
  3. Separately notify DDS, the case manager, and provider and use incident, grievance, correction, or appeal routes.

Expected next

A protective, law-enforcement, emergency, or DDS response plus a separate safety plan or service correction.

Save this proof

Report details and number if supplied, incident record, separate service concern, proof, responses, and corrections.

If this stalls or the answer is no

Use APS, law enforcement, 911, or the DDS grievance route according to the person, setting, and immediate risk; ask the case manager to own service correction.

Limits

APS reporting and service correction are separate. Protective jurisdiction, DDS incident review, appeal, law enforcement, and emergency response have different scopes.

Beyond Childhood’s checklists and prompts are planning aids derived from official sources. They are not official forms, legal advice, eligibility decisions, or guarantees of funding, capacity, quality, or service.
