Adult Protective Services

Before you start

  • Adult’s location, immediate safety, observable facts, dates, injuries or risks, alleged actor if known, witnesses, current services, and failed safeguards.

Who to contact

Use the official destination above, the county-assigned LME/MCO, the contact on the controlling notice, or the program-specific help route cited here.

Step by step

  1. For immediate danger call 911.
  2. Use the local DSS directory to report suspected abuse, neglect, or exploitation to APS in the county where the adult lives.
  3. Separately notify the provider and LME/MCO of the service breakdown and request a corrective plan.

Expected next

An APS report response when applicable plus a documented service-correction owner and next event.

Save this proof

Report details, agency contacted, reference if supplied, incident records, provider and plan notices, and corrective actions.

If this stalls or the answer is no

Escalate immediate danger to 911; follow up with county DSS and ask the LME/MCO who owns service correction.
