Human help at any point

Use the statewide HCBS waiver application route

Apply through iServe, submit the signed DD-10, or call 877-667-6266. HCBS Waiver Eligibility can identify the application owner; after eligibility and a waiver decision, an assigned service coordinator helps plan services.

A result, plan, or notice arrived

Separate every layer, build the person-centered plan, and use the notice-specific route.

Read the step-by-step guides
  1. Separate application, Medicaid, DD eligibility, level of care, interRAI, waiver and funding decision, service coordination, plan, authorization, provider or worker readiness, and start.
  2. Turn the person's goals, assessed needs, risks, services, safeguards, provider choices, and backups into the plan.
  3. Use the complete Notice of Decision and current fair-hearing route.

Choosing delivery, providers, payment, or a start

Compare agency and self-directed delivery and verify every operational layer.

Read the step-by-step guides
  1. Compare DD agency services with an independent provider and document each responsibility.
  2. Use the official agency and independent-provider directories, then verify service, capacity, safeguards, and fit.
  3. Separate the current fee schedule, person-specific funding and plan, authorization, operational readiness, and actual delivery.

A decision is adverse or decision support is needed

Use the correct hearing, grievance, benefits, housing, and decision-support routes.

Read the step-by-step guides
  1. Use the complete Notice of Action or Decision and the correct current fair-hearing route.
  2. Document the issue and use the coordinator, provider, grievance, complaint, or quality route without losing a separate appeal or safety report.
  3. Keep benefits and housing work moving while preserving the adult's legal rights and using the least restrictive support that fits.

JNY-NE-06

Urgent need, crisis, abuse concern, or service breakdown

Address immediate safety, protective reporting, reassessment, and service correction.

Read the step-by-step guides
  1. Call 911 for an extraordinary event needing immediate responders; otherwise call, text, or chat 988 for crisis support.
  2. Use APS, law enforcement, or emergency reporting as applicable and separately correct the failed service.
  3. Document changed need, failed supports, or risk and request the specific interRAI, waiver, plan, funding, authorization, provider, or crisis response.
