Request DORS servicesBefore you start
- For Pre-ETS only: age 14–21, enrollment in school or another approved education program, and documented disability are required.
- For employment services: gather available medical, education, and job-training records and a list of previous jobs. DORS will tell you what it needs.

Step by step
- If you have an IEP or 504 team, ask whether a DORS request has already been submitted. A school meeting is not required to submit a request yourself.
- Use the official Request DORS Services form or call the nearest DORS office for help. If you are not a student, ask about vocational-rehabilitation employment services.
- For Pre-ETS, follow DORS’s secure SARA instructions for disability documentation, proof of age and enrollment, and consent.
- Ask DORS which service route applies and what document or appointment comes next. Age alone does not establish student eligibility.

DORS asks you to allow 10 business days for the office to process the request and arrange an appointment.
