- For Pre-ETS: the student is age 14–21, is enrolled in school or another approved education program, and has documentation of disability.
- An email address, Social Security number, proof of age, proof of school enrollment, and disability documentation.

Step by step
- At the IEP or 504 meeting, ask whether the school has submitted a DORS request and obtain a copy or confirmation.
- If no request is confirmed, complete the official Request DORS Services form or call the nearest DORS office for help completing it.
- Watch for DORS’s secure SARA email and submit the requested qualification documents and consent.
- At the counselor meeting, ask whether Pre-ETS, Student Employment Services, or full vocational-rehabilitation services fit the student’s situation.
