Report suspected harm to Louisiana Adult Protective Services

Before you start

  • Immediate safety facts, age, location and setting, adult at risk, alleged harm, people involved, dates, witnesses, evidence, communication needs, plan and staffing facts, and callback information.

Who to contact

Use the Local Governing Entity, OIDD, Medicaid, Support Coordinator, provider or fiscal-employer agent, complete-notice appeal route, crisis service, or setting-specific protective-report route that owns this step.

Find the Louisiana Local Governing Entity

Step by step

  1. Call 911 if harm is occurring now.
  2. Use the current official route for age 18 to 59 APS, age 60 or older Elderly Protective Services, or a regulated facility; follow the page's telephone-only rule for physical or sexual abuse.
  3. Notify the Support Coordinator and applicable provider or incident owner when safe, and request a separate service correction.

Expected next

Protective response, report confirmation, safety action, investigation, incident review, service correction, or reassessment.

Save this proof

Report confirmation, evidence log, contacts, safety plan, incident records, and corrective plan.

If this stalls or the answer is no

Escalate immediate risk to 911; follow up separately with the protective owner, Support Coordinator, and Local Governing Entity.
