Step by step
- Review the official CHILD criteria: under age 21, the stated institutional level of care, exhausted other supports, and special Medicaid financial requirements.
- Apply through kynect or an ADRC or CMHC and save both Medicaid and waiver confirmations.
- If approved, track stabilization and begin written transition planning no later than 120 days before age 21, including whether MPW, SCL, RISE, or State Plan services may fit next.
