Step by step

  1. For immediate medical or safety danger call 911.
  2. For other behavioral-health or I/DD crisis support call or text 988 or use the regional CMHC crisis line.
  3. After immediate safety, notify the case manager and providers and request a documented follow-up plan.

A crisis response plus a named follow-up owner, assessment, plan update, referral, or emergency SCL action when applicable.
