Before you start

  • Person's age, location and setting, immediate safety, observable facts, dates, injuries or risks, alleged responsible person if known, witnesses, current services, and failed safeguards.

Start with DDDS Applicant Services or the assigned Community Navigator, Support Coordinator, or Family Resource Coordinator. Use the complete notice for review or appeal and the correct DDDS incident or APS route for safety concerns.

Step by step

  1. For immediate danger or a suspected crime requiring emergency response, call 911.
  2. Use the DDDS Incident Reporting System for the listed DDDS service settings or contact its administrator; use APS at 1-888-277-4302 for applicable vulnerable-adult community concerns.
  3. Separately notify case management and the provider and use service-correction or appeal routes according to the issue.
