Before you start

  • Medical, work, education, daily-function, income and resource records; SSA and housing records; adult preferences, trusted supporters, communication needs, decision areas, existing legal documents, and two eligible adult witnesses for an agreement.

Start with DDDS Applicant Services or the assigned Community Navigator, Support Coordinator, or Family Resource Coordinator. Use the complete notice for review or appeal and the correct DDDS incident or APS route for safety concerns.

Step by step

  1. Use SSA's official disability route and DSHA's official housing search as separate workstreams.
  2. Map the decisions where the adult wants help and the supporter roles and limits.
  3. If using a statutory agreement, follow the current written, dated, signature, witness, supporter-declaration, and revocation requirements and obtain legal advice when needed.
